AASA's appeals policy is that all bans must be appealed within 1 year from the ban, which you are well past. The MD5 is because a file was found on the system that shouldn't be there. If you still have the file on your system (ss.log) feel free to send it in, but with it being nearly 3 years later, I doubt you will find it. The file must match exactly (not just name, but also size and content) or the ban will be permanently denied.
